Completing the SBA Application

Please review the following notes before you begin filling out your SBA application, which is linked at the bottom of this page.

NAICS CODE:

NAICS codes explain what kind of business you are operating. Look up your exact NAICS code by using the search bar on this website. Be sure to search by using key industry words to find the right code: https://www.naics.com/search-naics-codes-by-industry/

APPLICANT OWNERSHIP:

We are requiring all owners to include their SSN in this section. Please do not use an EIN.

*If you have more than 2 owners with more than 20% stake in the company, it will exceed to 2 line limit and you will need to provide an additional document listing this information.

ADDENDUMS A & B:

If you answered "Yes" to question 3 or 4 on the application, please create a document titled Addendum A or B detailing the information requested. You will upload this along with your application in the Indy Chamber PPP Application.

EIDL outstanding amount at time of application

FRANCHISES:

If your business is a franchise and answered "Yes" to Question 8, please check to see if your franchise is listed in the SBA Franchise Directory. You will need to search the document and find the identifier code for Question 9 on the application. Pro tip: quickly search the Franchise Directory using clicking Clt+F on your keyboard to search rather than scroll.

For Schedule C Filers:

(single member LLCs, sole proprietors, and independent contractors)

NEXT STEPS:

1. Once you complete the application via the link above, you will receive a PDF copy of the document in your e-mail (please check your junk mail).

2. Save this document and move to the FINAL step of the application process by filling out this form (you will need your completed calculator, completed SBA application, and the documents listed in your PPP calculator).
